We are currently looking to add a Clinical Manager with Hospice experience to join our hospice team. Our service area includes Stanislaus, San Joaquin, and Merced Counties.

The Clinical Manager is responsible for ensuring that patient care is coordinated and managed appropriately. As well as ensuring that care and services are delivered appropriately and for the supervision of clinical personnel.

Essential Job Functions/Responsibilities:

1.Receives case referrals. Reviews available patient information related to case, including home visits, to determine hospice/home care needs. Assigns appropriate hospice personnel to case as needed. Conferences with MEDICAL DIRECTOR regarding any questions about an individual's eligibility for services.

2.Reviews and evaluates each case through a variety of means such as home visits, conferences, record review and the services provided by clinicians; discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services; and is available at all times during operating hours to assist clinicians as appropriate.

3.Reviews patient's medical diagnosis, prognosis, medications, procedures and clinical course.

4.Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care.
5.Attends case conference meetings with hospice personnel to facilitate coordination of care and discussion of interdisciplinary group involvement.

6.Conducts quarterly record reviews and communicates findings and recommendations to Clinical Director/Director of Patient Care Services and hospice personnel.

7.Assists in the screening and interviewing process of new employees and makes recommendations for employment of individuals. Assists in the orientation of new employees.

8.Assists Clinical Director/Director of Patient Care Services and other Managerial hospice personnel in the planning, implementation and evaluation of in-service and continuing education programs. Assists in the formulating, revising, implementing, and evaluating organization policies, procedures, goals and objectives, both short and long range.

9.Complies with accepted professional standards and principles.

10.Assesses health needs of community and assists in the development of plan(s)/ program(s) to meet needs. Participates in public relation and community activities that promote the organization's role as an effective member of the hospice and health care delivery system. Promotes customer service orientation to all hospice personnel.

11.Participates actively in quality assessment performance improvement teams and activities.

12.Performs other duties and activities as delegated by the Clinical Director/Director of Patient Care Services.

JOB GOAL:

Direct, coordinate and evaluate the delivery of home health or hospice services to a team of patients ensuring compliance and consistency with agency policy, State and Federal home health regulations. Monitors attainment of the objectives of the team activities.

JOB RESPONSIBILITIES:

・Provides direct clinical supervision, performance evaluations and disciplinary action of all assigned team staff. Obtains input from specialty manager (Rehab Supervisor/Home Health Aide Supervisor) where appropriate. Evaluates and ensures that competency requirements are met for all staff

・Ensures and performs in-home supervisory visits with assigned team members

・Monitors staff assignments, caseloads, and team productivity. Promotes efficient utilization of resources to achieve desired clinical outcomes and within episode reimbursement

・Develops and maintains a system of patient tracking and financial forecasting. Ensure OASIS review and coding results in appropriate reimbursement for the patient’s clinical condition. Refers and discusses high utilization and problem prone cases with Director of Clinical Services and Quality Manager.

・Provides the Director of Clinical Services with census, staffing, team productivity and other reports as requested. Identifies staffing patterns and needs of assigned team.

・Implements and supports field Case Management philosophy of care.

・Facilitates appropriate communication and documentation between patient, team members, medical professionals, and referral sources.

・Completes initial Plan of Care review with Case Manager in a timely fashion with a focus on quality, utilization, and individualized patient care and goals.

・Conducts individual caseload reviews and team meetings on a regularly scheduled basis according to the agency’s standards. Case conferences with staff as needed to meet the needs of challenging clients.

・Participates with the Director of Clinical Services in interviewing, screening, and selecting candidates for team positions.
Takes on call when necessary.

・Acts as a role model and a mentor professionally and clinically for the implementation of the organization’s Mission and Value Statements.

・Ensures staff orientation and development programs are enacted.

・Represents the agency on community committees/professional organizations and increases community awareness of agency in selected public relations efforts.

・Performs other duties as assigned.

Salary Range 【Full-time】 $110,000.00 〜 $145,000.00 /year

Requirements

・Registered nurse with current license to practice in the state of California
・A minimum of one (1) year experience as a professional nurse within the last three (3) years.
・Previous experience in hospice/home care setting and with two (2) years management or Managerial experience. Proven ability to work within an interdisciplinary setting.
・Complies with accepted professional standards and practice.
・Has excellent observation, good nursing judgment and communication skills. Understands hospice philosophy and issues of death/dying.
・Understands principles of pain/symptom management.
・Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
・Complies and maintains current CPR certification.
