Under the supervision of the SWSA Palliative Care Nurse Manager, and with the ability to work independently and proficiently, provides direct clinical support and education for Palliative Care services in the Inpatient setting. Actively collaborates with members of the palliative care team: Inpatient and Outpatient providers, Nurses, Social Workers, Pharmacist, and Chaplain in the provision of quality Palliative Care. The Palliative Care Nurse identifies and triages patients that may benefit from palliative care. The Palliative Care Nurse facilitates palliative intervention/ end-of-life discussion and care, responds to referrals/consults, and participates in family meetings and goals of care conversations. S/he educates/mentors nurses, clinicians, and non-clinicians in evidence-based palliative care.

Required Qualifications:
Nursing degree/diploma required upon hire
Bachelor's Degree Bachelor's in Nursing or higher within 3 years of hire Or equivalent educ/experienceA RN with 20 years' of RN experience will not be required to obtain a BSN
RNs employed prior to 1/1/2017 are encouraged to obtain a Bachelor's degree or higher in Nursing, but obtaining a degree will not be a condition of employment
Any previous agreed upon effective date for obtaining a Bachelor's degree in Nursing between individual and facility will remain in effect
Coursework/Training Attain Hospice and Palliative Care Nursing (CHPN) Certification within 2 years of hire
Upon hire: National Provider BLS - American Heart Association
Within 2 years of hire: Actively work to complete Hospice or Palliative Care Nursing Certification
3 years Recent clinical experience in an acute care setting with a background in medical, oncology, or hospice nursing.
Preferred Qualifications:
3 years Experience in Palliative care

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
