Position Overview:

Plans, develops, coordinates and directs the operation of the Pathway to Excellence program through the American Nurse Credentialing Center. Oversees the execution of procedures and adherence to proper protocols and develops and/or administers performance improvement initiatives for the program. Assumes responsibility for program quality and compliance, and the education and competency of nursing staff. Serves as a clinical and educational consultant to the nursing staff, medical staff, management, patients and other members of the health care team regarding the program. Provide direct patient care as needed on an irregular basis.

Job Description :

JOB ACCOUNTABILITIES:

1. INTEGRATION OF PATHWAY STANDARDS

1.1 Facilitates nursing leadership understanding of and integration of Pathway to Excellence standards.

1.2 Promotes departmental strategic planning that addresses implementation of standards.

1.3 Facilitates periodic self-assessment / gap analysis and improvement planning to strengthen integration of standards.

1.4 Stays appraised of the American Nurse’s Credentialing Center’s (ANCC) revisions and clarifications regarding standards.

1.5 Communicates revisions and clarifications to nursing leadership and other appropriate personnel.

2 PATHWAY OFFICE LIAISON

2.1 Maintains open communication with ANCC’s Pathway to Excellence Office staff.

2.2 Confirms / communicates changes and revisions in ANCC Pathway policies and procedures.

2.3 Maintains accurate and current files of all ANCC correspondence.

2.4 Once Pathway designated, coordinates all required activities with ANCC and responds to queries from other organizations seeking Pathway designation.

3 EDUCATION; COMMUNITY RELATIONS

3.1 Develops and coordinates Pathway-related educational materials and presentations for all employees.

3.2 Mentors staff nurses to support their role as educators regarding the Pathway process.

3.3 Develops and coordinates the Central Shared Decision-Making Team.

3.4 Provides ongoing education about the Pathway standards and principles of a professional practice environment to support the goals of Asante’s Strategic Plan.

3.5 Serves as an educational resource to the nursing community regarding the Pathway process.

3.6 Works closely with Communications and Marketing to produce Pathway-related publicity / advertisement materials.

3.7 Coordinates celebrations and award ceremonies and displays in collaboration with Communications and Marketing.

4 APPLICATION / RE-APPLICATION PROCESS / DATA MANAGEMENT

4.1 Chairs and coordinates meetings and activities of the Pathway to Excellence Team.

4.2 Ensures timely compliance with procedures for submission of all materials required for application / re-application.

4.3 Coordinates efforts to identify appropriate evidence for each standard / indicator.

4.4 Coordinates collection, categorization, and filing of all Pathway evidence.

4.5 Coordinates preparation, editing, duplication, and publication of all Pathway evidence as it pertains to the standards.

4.6 Prepares periodic reports and updates on progress toward designation / re-designation.

4.7 Once Pathway designated, prepares and files all required on-going documentation.

5 INTRA/INTER DEPARTMENTAL COLLABORATION

5.1 Collaborates with department managers to educate staff.

5.2 Provides periodic updates to managers on the Pathway process.

5.3 Facilitates integration of Pathway standards as needed across all departments.

5.4 Collaborates with the accreditation Coordinator to assure integration of Pathway standards as they relate to any regulatory standards.

6 DATA MANAGEMENT

6.1 Oversees accurate demographic and statistical data collection for all Interim Monitoring Reports.

6.2 Contributes information to develop/strengthen the Pathway organization database.

EDUCATION:

  • Graduate of an accredited school of nursing
  • Bachelor's degree in nursing.
  • Preferred: Master's degree in nursing or another applicable field.

CERTIFICATION & LICENSURE:

  • RN-Registered Nurse of California
  • BLS-Basic Life Support Healthcare Provider

TYPICAL EXPERIENCE:

  • 3 years recent relevant experience.
  • Project Managment experience in a leadership role

SKILLS AND KNOWLEDGE:

  • Expert knowledge and competence with regard to nursing theories, concepts and practices; medical terminology; anatomy; and physiology.
  • Knowledge of the healthcare industry, safety precaution policies, best practices regarding patient care and privacy, and changes in local/state/federal regulations.
  • Knowledge relating to healthcare delivery, project management, and quality improvement.
  • Experience defining, configuring, accessing, querying, and validating data.
  • Ability to work independently; receives minimal guidance.
  • Written and verbal communications skills to explain sensitive information clearly and professionally to diverse audiences, including non-medical people.
  • Knowledge of computer applications, such as Microsoft Office Suite (Word and Outlook), Electronic Health Record (EPIC preferred).
  • Prioritize assignments and work within standardized policies, procedures, and scientific methods to achieve objectives and meet deadlines.
  • Work independently, as well as be part of the team, including accomplishing multiple tasks in an environment with interruptions.
  • Identify, evaluate and resolve standard problems by selecting appropriate solutions from established options.
  • Ensure the privacy of each patient’s protected health information (PHI).
  • Build collaborative relationships with peers and other healthcare providers to achieve departmental and corporate objectives.

Pay Range is $83.28 to $109.07 / hour

The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs. Base pay is just one piece of the total rewards program offered by Sutter Health. Eligible roles also qualify for a comprehensive benefits package.
