Description We are looking for a Registered Nurse (Case Manager Experience) for our team in Chicago, IL!

$80,000-$90,000

Monday-Friday, 8am-5pm

No on-call

No weekends

Additional Details

- A lot of field work (mileage reimbursement provided).

- When not in field, will be in Chicago office doing virtual visits or also doing work at home (company laptop provided).

Job Summary

The Registered Nurse (Case Management) role involves a mix of field work and virtual visits. The RN is expected to be self-motivated and require minimal oversight. Key responsibilities include supporting census management, reviewing patient data, coordinating transitions of care and post-discharge planning with healthcare providers, as well as potentially conducting outreach to patients by phone. The RN will play a crucial role in new patient engagement and overall care coordination.

Qualifications

- IL RN license.

- BSN degree.

- At least 5 years RN experience.

- At least 3 years of care management experience in the health plan, home health, and/or hospice.

- Case Management experience on the provider or payor side with a focus on patient interaction.

- Passion for helping underserved population.

Pay Details: $80,000.00 to $90,000.00 per year
