The Clinical Program Coordinator is responsible for the planning and coordination of the Swedish Aortic Center (SAC) within the Swedish Heart & Vascular Institute (SHVI) throughout the entire patient experience including patient referral intake, follow-up coordination, patient care progression facilitation and tracking, patient database management, and quality and operational metrics tracking. The coordinator role is designed to optimize patient, staff and physician integration within the SAC program at Swedish, a joint program between Cardiovascular Surgery and Vascular Surgery specialties. They will also serve as the Liaison with other programs within Swedish, Providence and other local/regional hospitals. The Coordinator is also responsible for providing support in the development, implementation of other programs as they relate to SAC at Swedish & SHVI, such as Cardiology, Primary Care, Emergency and Acute Care, but not limited to these programs.Providence caregivers are not simply valued – they’re invaluable.   • Assist patients in understanding their diagnosis, treatment goals, and the resources available; including educating eligible patients on appropriate clinical research studies and technologies.
  • Responsible for the administrative and operation components of the SAC program. Serves as a Dyad to the SAC Clinical Lead(s). Will also work with SHVI Executive Medical Directors, Senior Director of Operations, participating physicians and staff to ensure that the SAC program needs are understood and met for a successful program.
  • Facilitate appointments for consults and support services that are important to the well-being of this patient population including other clinical specialties and those services that may facilitate care and recovery outside the hospital environment.
  • Track emergency and hospital visits for patients of the clinic and facilitate timely follow up.
  • Work with clinical staff to develop and use evidence-based care approaches to help improve clinical outcomes and reduce emergency and hospital visits.
  • Support and help implement the program’s aortic disease pathway, while ensuring that other care team disciplines remain abreast of the pathway and its goals.
  • Encourage and champion patient engagement in their treatment plans and goals.
  • Leads the SAC program implementation and operational team meetings. Ensures the timeline and key deliverables of the implementation plan are achieved.
  • Coordinate multidisciplinary planning conferences, develop concise patient summaries for use by the care team, and document recommendations made utilizing standardized care protocols in accordance with nationally recognized care guidelines.
  • Track and deliver treatment and outcomes data and present results quarterly to the appropriate committees and groups.
  • Assist in outreach efforts to establish and maintain positive working relationships with key stakeholders (physicians, advanced practice providers, office staff, administrators, diagnostic staff, nurses, pharmacists, social workers, allied health providers, etc.)
  • Accountable for the key performance measures of the SAC program as identified by clinical leads. Works with the team to identify and track key performance measures including but not limited to: patient satisfaction, physician satisfaction, LOS, cost/case, discharge disposition, and aortic disease specific outcome tools as necessary. Works with the data informatics to distribute reports to key stakeholders and participates in process improvement initiatives as required.

Required Qualifications:

  • Bachelor's Degree Graduate of an accredited Bachelors Nursing degree program or completion of an accredited Nursing program with a Bachelors degree or higher in a health care field. Applicants without a BSN degree must actively pursue and attain a BSN within three years of hire. Continued employment by Swedish beyond three years is contingent upon attaining the BSN degree.
  • Upon hire: Washington Registered Nurse License
  • Upon hire: National Provider BLS - American Heart Association
  • 5 years Professional nursing experience in Cardiology, with demonstration of competent clinical judgment and clinical leadership ability.
