About the Role: As a field RN, you will work in a hybrid position that combines virtual case management with occasional field visits. You will support the coordination of care for a diverse patient population, primarily Medicare recipients aged 65 and older. Your role will involve managing patient transitions, engaging with high-risk patients, and building strong relationships with key partners, such as hospital case management teams and primary care providers. Key Responsibilities: Case Management: Oversee patient care coordination, manage census, and review daily reports from inpatient, ER, and skilled nursing facilities. Patient Outreach: Engage with patients telephonically, facilitate new patient onboarding, and support patient care transitions. Collaboration: Work closely with healthcare partners, including hospital case managers and other clinical team members, to ensure smooth transitions and effective patient care. Program Development: Help establish and standardize care management processes in line with organizational growth and goals. What We Offer: Competitive Salary: $86,000 - $90,000 per year Comprehensive Benefits: Including medical, dental, vision, HSA, Life and Disability, FSA, and 401k offered!
