Location: Hybrid Role in Long Beach, CA (Virtual and Field-Based)

About the Role:

As a field RN, you will work in a hybrid position that combines virtual case management with occasional field visits. You will support the coordination of care for a diverse patient population, primarily Medicare recipients aged 65 and older. Your role will involve managing patient transitions, engaging with high-risk patients, and building strong relationships with key partners, such as hospital case management teams and primary care providers.

Key Responsibilities:

  • Case Management: Oversee patient care coordination, manage census, and review daily reports from inpatient, ER, and skilled nursing facilities.
  • Patient Outreach: Engage with patients telephonically, facilitate new patient onboarding, and support patient care transitions.
  • Collaboration: Work closely with healthcare partners, including hospital case managers and other clinical team members, to ensure smooth transitions and effective patient care.
  • Program Development: Help establish and standardize care management processes in line with organizational growth and goals.

What We Offer:

  • Competitive Salary: $86,000 - $90,000 per year

  • Flexible Schedule: Monday to Friday, 8 am - 5 pm, with no weekends or on-call requirements

Qualifications:

  • Active CA RN License and BSN required
  • Minimum of 5 years of relevant clinical experience
  • At least 3+ years of care management experience in health plans, home health, and/or hospice preferred
  • Background in case management essential
  • Must be flexible, adaptable, and willing to collaborate in a growing and evolving environment
  • Strong interpersonal skills with the ability to build and maintain professional relationships with key stakeholders
