Description Cheney Care Center is seeking a PRN COTA to join our team!
Cheney Care Center is the only community founded skilled nursing facility in the state of Washington. We are a not-for-profit facility dedicated to providing high quality post-acute rehab and long-term care.
Specific Requirements/Expectations:
I. Education: All COTAs must have graduated from an accredited education program with the appropriate degree.
II. Licensure: All COTAs are responsible for having up-to-date licensure and copies of state licensure for the state(s) in which they work.
III. Driver's License: All COTAs must have an up-to-date state driver's license for their state of residence.
IV. Service Provision:
All COTAs will:
Treatment
1. Ensure that a clarification order is in the chart prior to initiating treatment if requested by the facility prior to providing treatment. If not required, ensure that there is an order for treatment included with the order for evaluation. (I.e. Eval and treat as indicated).
2. Ensure that the Plan of Treatment is written and validated before the COTA provides treatment.
3. Ensure that insurance authorization has been obtained.
4. Provide treatments as outlined on the plan of treatment and within their scope of practice and level of experience.
5. Report changes in status, function, or medical condition to the supervising therapist.
6. Communicate regularly and as needed to other team members and on-call therapists re: patient care issues.
7. Train staff and family as directed by supervising therapist.
8. Coordinate scheduling issues with team members and the Rehab Director as needed.
9. Manage time and prioritize responsibilities efficiently, with consideration given to the needs of others (professionals, patient, families, etc.), and productivity expectations.
10. Meet and/or exceed productivity standards.
Screenings
1. Provide input on screens as directed by the supervising therapist.
Documentation
1. Complete all required documentation in a timely manner (to include daily activity and labor logs, treatment encounter notes, where allowable by regulation, weekly summaries, discharge reports, and any other facility specific documentation), as per scope of practice.
