Other States in USA00 Must commit to 4 days per month Must provide all required documentation to comply with hospital requirements Drug test is required Background check required RN License BLS/ACLS/PAL as required for type of Hospital floor unit Proof of Vaccines (Tdap, Flu, MMR, Hep B, TB test, Varicella etc.) or Immunity Blood test Must pass proficiency test and acknowledgment forms 2 forms of ID |
