UW Medical Center-Montlake – Alvord Brain Tumor Center has an outstanding opportunity for a Per Diem Oncology Nurse Navigator (job profile: Registered Nurse 2).

WORK SCHEDULE

  • Per diem work schedule
  • Must be available to work 8 hour day shifts Monday - Friday
  • Preference will be given to RNs who can regularly work up to 2 days each week
  • Our clinic is closed on nights, weekends, and holidays DEPARTMENT DESCRIPTION The Alvord Brain Tumor Center provides expert and holistic care that includes a precise diagnosis, a personalized care plan, and a team of experts who guide patients each step of the way from consultation to treatment and follow up. Our care team includes neuro-oncologists, neurosurgeons, radiation oncologists, neuropathologists, oncology nurses, and cancer specialists who work together to treat brain and spinal cord cancers. (https://uwmedicineneuro.org/brain-tumor) PRIMARY JOB RESPONSIBILITIES
  • Provide patient-centric care for a breadth of neuro-oncologic diagnoses, including brain and central nervous system (CNS) tumors
  • Assist patients, families, and caregivers to overcome healthcare system barriers
  • Work between the domains of the patient and family and the healthcare delivery system to improve health, treatment, and end-of-life outcomes
  • Provide care coordination throughout the cancer continuum, ensuring timely access
  • Provide education and resources to patients, families, and caregivers about diagnoses, treatment options, side effect management, post-treatment care, and survivorship
  • Assess and promote healthy lifestyle choices and self-care strategies through education and referrals to ancillary services
  • Develop and use appropriate assessment tools (e.g. distress screening, pain scale, performance status)
  • Support a smooth transition of patients from active treatment into survivorship or end-of-life care
  • Demonstrate knowledge of the Fred Hutch and UWMC Clinical Pathways
  • Facilitate communication among members of the multidisciplinary cancer care team to prevent fragmented or delayed care that could adversely affect patient outcomes; this includes participation and coordination of multidisciplinary case reviews and tumor boards REQUIREMENTS
  • 3 years of recent care management nursing experience, oncology nursing experience, or equivalent experience
