Description Summary:

The Certified Nurses' Aide completes specialized training in areas of leadership, mentoring and person-centered care. Nursing care and nursing related services are provided to residents and patients according to each resident's/patient's comprehensive assessment and plan of care. All resident/patient care is provided in a manner that meets the resident's/patient's physical, mental and psychosocial needs and enables the individual to attain and maintain the highest practical level of functioning.

Essential Duties and Responsibilities:

Include the following: Identifies and suggests innovative approaches and participates in implementing performance improvement opportunities. Maintains compliance with Presbyterian SeniorCare Network policy and procedure in accordance with governing laws, regulations, regulatory agencies, accreditation bodies and community standards. Demonstrates on a consistent basis individual and team behavior that supports the Person-Centered, Woodwell and Woodside philosophies of Presbyterian SeniorCare Network. Consistently upholds the mission, values and behavior standards of Presbyterian SeniorCare Network. Maintains confidentiality. Other duties may be assigned.

1. Following each resident's/patient's plan of care provides direct care to improved or maintain resident's/patient's abilities.
2. Performs nursing rehabilitation techniques as taught and approved by the community to promote and maintain the health status of the resident/patient.
3. Assists residents/patients with meals and nourishments. Monitors food and fluid consumption in order to identify dietary concerns.
4. Assists residents/patients with transportation to activities and appointments within the community to ensure residents safety and timely arrival.
5. Takes accurate vital signs and weights. Notifies the nurse of changes.
6. Participates in lifestyle engagement programs as requested.
7. Exhibits excellent customer service behaviors and interactions at all times.
8. Follows and promotes infection control policies and procedures. Reports occupational exposures to blood, bodily fluid, infectious materials and hazardous chemicals in accordance with established policies and procedures.
9. Records accurate and legible information about resident/patient care and condition in appropriate sections of the resident's/patient's record.
10. Communicates often with manager and/or nurse about residents'/patients' status throughout the shift.
11. Annually attends/completes required in service training provided by the community. Applies information to job tasks and duties.
12. Floats throughout the community as assigned.
13. Answers call lights promptly.
14. Performs restorative functions as instructed.
15. Uses supplies in a cost conscious manner.

Education and/or Experience:

Certificate in good standing from Nurse Aide Registry. Minimum age of 18.
