CARE TEAM NURSE (RN/LPN)

Location: West Salem Clinic, 1233 Edgewater Street NW, Salem OR 97304

Job Status: Full-time, Monday-Friday with rotating Saturdays

Sign-Bonus : $3,000 (Internal candidates that are currently employed here at NWHS are not eligible for a sign-on bonus.)

Referral Bonus: $1000 (Referral bonus’ expire at the close of this job posting.)

YOUR ROLE:

As our Care Team Nurse, you will use a trauma-informed and whole-person approach to address patient wellness, acute illness, chronic disease, disability, and end-of-life needs in collaboration with their assigned primary care provider and care team. In this role, you will educate and motivate patients in areas related to self-care; dispense telephone advice to patients with minor acute problems; support the ongoing care management of patients with stable chronic disease; monitor patients during routine office visits; and perform of a variety of procedures that are within the scope of the professional nursing practice.

QUALIFICATIONS:

  • Oregon Nursing License (RN or LPN)
  • CPR/BLS certification
  • Possess clinical expertise in an outpatient environment and triage.
  • Ability to exercise independent judgement and decision-making.
  • Ability to prioritize competing responsibilities while under pressure.
  • Demonstrates problem solving skills.
  • Ability to manage high volumes of patients in a short span of time, while dealing with unpredictable issues.
  • Ability to work with diverse cultures and lifestyles.
  • Bilingual English/Spanish preferred.
