Description LPN - Home Health-SCS Home Health Schedule: Per Diem, Days, minimum of 4 shifts a month Floating between Thurston, Pierce, Mason and Lewis County Licensed Practical Nurses (LPNs) work alongside an interdisciplinary team to provide whole-person quality care to patients in need. Required Qualifications: Graduate of a school of nursing Upon hire: Washington Licensed Practical Nurse Within 30 days of hire: National Provider BLS - American Heart Association Within 30 days of hire: National Provider Identifier (NPI) number Upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information. 1 year Nursing experience in an acute care setting This position requires that you obtain an NPI number within 30 days of starting in the position. About the Team Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint. HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses.
