Job Summary:

We are seeking a dedicated Licensed Practical/Vocational Nurse (LPN/LVN) to provide skilled nursing care to patients in their homes on an intermittent basis. This role operates under the direction and supervision of a Registered Nurse (RN), following physician orders and care plans in line with our Agency’s policies.

Qualifications:

1. Graduate of a state-approved school of practical (vocational) nursing and currently licensed in the state(s) of practice.

2. Minimum of one (1) year of nursing experience preferred.

3. Alignment with the philosophy and goals of our Agency.

4. Ability to exercise initiative and independent judgment.

Responsibilities:

1. Adhere to established policies and procedures.

2. Implement nursing care plans for each patient.

3. Provide nursing services, treatments, and diagnostic and preventive procedures as assigned.

4. Initiate preventive and rehabilitative nursing procedures as appropriate.

5. Observe signs and symptoms, reporting reactions to treatments to the physician and RN.

6. Teach and counsel patients and their families regarding nursing care needs and related problems at home.

7. Evaluate the effectiveness of the LPN/LVN’s nursing service with the RN.

8. Maintain accurate and complete records of observations, treatments, and patient care.

9. Participate in medical record audits as assigned.

10. Attend staff meetings, patient care conferences, and in-services as scheduled.

11. Take on-call duty, including nights, weekends, and holidays as assigned.

12. Submit schedule changes to the Director of Clinical Services/Nursing Supervisor daily.

13. Participate in patient care conferences for team collaboration.

14. Prepare clinical and progress notes.

15. Assist physicians and RNs in performing specialized procedures.

16. Prepare equipment and materials for treatments.

17. Observe aseptic technique as required.

18. Assist patients in learning appropriate self-care techniques.

Working Environment:

  • Work indoors in Agency offices and patient homes.
  • Travel to/from patient homes.

Job Relationships:

  • Supervised by: Director of Clinical Services/Nursing Supervisor/RNs

Lifting Requirements:

  • Ability to participate in physical activity.
  • Ability to work for extended periods while standing and being physically active.
  • Heavy lifting.
  • Extensive bending, lifting, and standing regularly.
