Requirements	Basic life support Driver’s license Licensed dentist Eaglesoft Emergency Dentistry General Dentistry DEA Registration Nitrous Oxide Certification Experienced Dentist (2+ years) Tooth Extractions Minimum 2 years of clinical experience Active Kentucky Dental License Proficiency in: Extractions Restorative dentistry Crown and bridge Removable prosthodontics Premolar and anterior endodontics
