Responsibilities

The Nurse Practitioner - Support Team for Access and After-care Resources (STAAR) position, provides high-quality care to patients and support to physicians. They do this by providing care for existing patients with acute and

virtually appropriate needs, expanding application of asynchronous care, and reducing administrative and clinical burden to in-clinic PCPs and Advanced Practice Providers. This position works within the context of a medical home, from a team approach, and in continuous partnership with PCPs and patients to promote timely access to needed care and the enhancement of patient and family well-being. The STARR NP in this setting is expected to know his/her scope as defined by the California Board of Nursing (NP) and functions at the top of his/her licensure. The STAAR NP is employed by Dignity Health/Pacific Central Coast Health Centers (PHC) , working in alignment with CommonSpirit Health’s

goals and initiatives. Goals include increasing wellness, preventing illness, and improving outcomes through early detection, wellness visits and timely diagnosis or treatment. The STAAR NP works with the extended care team to ensure the patient experiences a smooth transition across the continuum of care.The STAAR NP ensures smooth transitions between and across care teams and settings. STAAR NP is a crucial member of the team, working under the supervision and clinical leadership of the STAAR Program Physician Lead and the direction of PHC.

Qualifications

1 year clinical experience including acute care, outpatient community health clinics, private physician offices, home health and hospice, public health setting.

Graduate of an accredited schools of nursing. Master's degree in nursing. Nurse practitioner certification from an accredited association.

Must be able to meet credentialing requirements.

RN; CT NP; DEA; CT CPRBLS

Preferred:

3 years experience as a NP in ambulatory community involving care management and/or chronic disease management (including outpatient community health clinics, private physician offices, home health and hospices, public health

settings, free-standing urgent care clinics) or primary care setting.

Experience in care coordination and chronic disease management preferred.

Pay Range

$59.53 - $86.32 /hour
